Log into Service Request Management in Remedy

https://request.umsystem.edu/arsys/shared/login.jsp?/arsys/forms/kc-itsm-plapp/SRS%3AServiceRequestConsole

Click the Help icon  from the menu in the upper right of window.

Select the “Complete Survey” option from the drop down menu

The survey window will appear with the most recent survey selected. You can change to any survey by

Clicking in the “Select a Survey from the list, then complete and submit it.” Drop down list. Complete the

Survey questions and click the “Submit and Close this request” button.
